Consent and Release
If you submit a Testimonial to TRC using the above form, then you agree to the following:
I give my permission to, and authorize, TRC Healthcare (hereinafter “TRC”), its employees, designees, agents, independent contractors, subsidiaries and affiliates, successor and assigns or others acting on behalf of TRC to, in TRC’s sole discretion, use, publish, reproduce, exhibit, and display my Testimonial as well as my name, image, photograph, and any other information provided by me in the above form for marketing, promotional, advertising of TRC’s products and services and/or other lawful commercial purposes in any marketing channel including, without limitation, through TRC’s website, or a website of any TRC affiliate, through social media outlets and in or through other electronic, print or digital materials or channels without notification or compensation to me.
TRC may edit my Testimonial and publish edited or partial versions of my Testimonial with the understanding that TRC will not edit my Testimonial in such a way as to create a misleading impression of my views. Any photo or image submitted by me may be cropped or edited for publication.
The rights granted herein are irrevocable and extend throughout the world. I confirm that (i) the Testimonial provided is an honest and accurate representation of my opinion and experience with TRC's products/services; (ii) the Testimonial is my original statement, written by me; (iii) I am 18 years of age or older and (iv) I have the right to contract in my own name.
TRC reserves the right to use or publish any Testimonial, photo or image in its sole discretion.
